Text seems the obvious choice. Is this a diary or an autobiography? Honestly, the difference seems mostly to be perspective.
2. Outline a few things that will be recorded. (No need to be detailed right now).
-Early life, before what's-its-name showed up. She'll be glossing over the details of this, but she still needs to include enough to make it clear why she was so eager to accept that deal.
-Euphoria-fueled wild years. For a few years after her transformation, she, uh, got salacious. Check with BOB-B who the target audience for this document is and censor accordingly.
-Wandering years. After calming down from the wild years, she just wandered the dimensions as an adventurer, doing whatever odd jobs she came across. This might've continued indefinitely, if not for...
-Whatever the fuck was up with that very brief employment at MAE.

That's... a very good question, actually. If Charis is removed from the time stream, wouldn't it follow that anything she made would vanish too? Check with BOB-B on relevant research and countermeasures. But for starters, magic might help. Start with time, an inverted null, a protection... and go on until she can't think of any more runes to add. A spell this important, you don't want to rush. And once the spell is designed, apply it liberally - get as many secure containers as possible, of as many different types as possible, and store a copy of the book in each one, cast the spell, and stash them away in various safe places throughout the dimensions... or as far throughout the dimensions as BOB-B will allow her to go.
